Log in to ServiceM8
Go to Settings
Go to Forms
Go to Import File
Find the File and Upload

Once uploaded you will find the Form in your Forms List, if you want to edit the file name you can.
 
Changes can be made to the following questions (when in Edit mode)
Date of Insurance Policy Expiry: Change from text to multiple choice and plumber licence no.
 
Do not change any other questions or answer as this would interfere with the template.
